The current model offers two ways to begin: a 60-minute Initial Adult Primary Care Evaluation for a broader review of health priorities, relevant history and medications, preventive needs, available laboratory information, and an individualized initial plan; or a 30-minute Focused Adult Primary Care TeleVisit for one primary health concern that may be appropriate for telehealth.
The purpose is clarity: understand what matters now, what may require follow-up, and what the appropriate next step should be.

Review the appointment instructions and be ready for your scheduled TeleVisit. You must be physically located in Florida at the time of the visit.

Attend Your Scheduled Visit
Your visit will follow the appointment type you selected: a broader 60-minute Initial Adult Primary Care Evaluation or a 30-minute Focused Adult Primary Care TeleVisit for one primary health concern that may be appropriate for telehealth.
Meet Héctor Silva, APRN, FNP‑C
Héctor Silva brings broad clinical experience and an approach centered on adult health, prevention, careful evaluation, and clear clinical decision-making. His background includes medical education and clinical experience in Cuba, followed by nursing and nurse practitioner training in the United States.
He provides care in English and Spanish and values clear communication, clinical context, thoughtful decisions, and respectful professional relationships.

Create an individualized care plan with clear next steps, which may include follow-up, testing, referral, in-person evaluation, or another appropriate level of care.
Telehealth is used only when clinically appropriate. If your visit determines that you need an in-person examination or other diagnostic testing, Silva Skin Solutions will guide you on the appropriate next step. Some services or levels of care—such as urgent care, emergency care, or specialist evaluation—may require care outside Silva Skin Solutions.
A Defined Initial Episode
The Initial Adult Primary Care Evaluation is one of two ways to begin with Silva Skin Solutions and is designed for a broader review of your health priorities.
Initial Evaluation
A 60-minute initial evaluation focused on understanding your priorities and creating an individualized initial plan.
Follow-Up When Clinically Needed
One 30-minute follow-up TeleVisit may be included to review laboratory testing ordered by Silva Skin Solutions or to follow up on the initial plan when clinically appropriate.
When Separate Care May Be Needed
New concerns, outside laboratory testing, additional testing cycles, or continued management may require a separate visit or another appropriate level of care.
The initial episode does not automatically establish ongoing or unlimited primary care.

The practice does not promise immediate response, 24/7 access, unlimited messaging, guaranteed prescriptions, or guaranteed continuation of medications prescribed elsewhere.
